Extremely painful hemorrhoids?

I’ve had this problem for about 2-3 years now and it doesn’t seem to be getting any better. I went to the doctor; who is COMPLETE idiot! my god, I told the woman I’ve tried everything from fiber, preparation H and stool softeners. She prescribed me stool softeners after I specifically told her that I don’t need stool softeners because I’m having diarrhea. I have a bowel movement about once a week and when I go, I really go. the first stool is really long and hard. After that. it’s nothing but diarrhea. Imagine having your anus rubbed with chili and set on fire. (sorry for the graphical image) It sometimes gets so bad I vomit. The problem is they only show up when I have a bowel movement. So by the time I go to the doctor, you can’t see them. What should I do?
